During our lifetime we all experience transitions; times when we journey from one stage to another. These transitions range from chronic illnesses to unexpected health concerns to the normal processes of aging. Geriatric Care Managers can help guide families through these transitions.

The Mid-Atlantic Geriatric Care Managers Association is an organization of private practitioners whose goal is to advance the dignified social, psychological, and health care for older adults, others with chronic needs, and their families. Members conduct their practices in Delaware, Maryland, Virginia, West Virginia, and the District of Columbia.

Geriatric Care Managers (GCMs) are professionals who encourage independence and autonomy of at risk elders. The association and its members strive to ensure that the highest quality and most cost-effective health and human services are used. We are very knowledgeable regarding resources in the Mid-Atlantic region. Our goal is to provide comfort and improve the quality of life for the elderly while relieving the stress from caring families.
