Welcome To Mid-Atlantic GCM

The Mid-Atlantic Professional Geriatric Care Managers Inc. is an organization of private practitioners whose goal is to advance the dignified social, psychological, and health care for older adults, others with chronic needs, and their families. Members conduct their practices in Delaware, Maryland, Virginia, West Virginia, Pennsylvania, and the District of Columbia.

Mid-Atlantic Professional Geriatric Care Managers (GCMs) are professionals who encourage independence and autonomy of at risk elders. The association and its members strive to ensure that the highest quality and most cost-effective health and human services are used. We are very knowledgeable regarding resources in the Mid-Atlantic region. Our goal is to provide comfort and improve the quality of life for the elderly and persons with disabilities, while relieving the stress from caring families.

What We Do

The Geriatric Care Manager receives inquiries from adult children or other family members (including those living at a distance from their elders), and referrals from attorneys, hospitals, nursing homes, physicians, community agencies, employee assistance programs, and the general public.

Throughout the elder care process, many find it helpful to bring in a professional with expertise in senior care issues. A Geriatric Care Manager specializes in assisting older people and their families with long-term care arrangements. Private care managers remain independent from the resources they recommend in order to provide an unbiased assessment of the situation.

We can help you meet the challenges of long distance caregiving, put together a comprehensive plan for present or future needs, provide extra assistance for relocation, or monitor your relative during your vacation.

Geriatric Care Managers can:

  • Conduct care-planning assessments to identify problems, and need for services.
  • Coordinate and monitor senior services, such as home care.
  • Act as a liaison to families at a distance.
  • Offer counseling, support and advocacy.
  • Provide information, referral and placement.
  • Offer crisis intervention and care management services.
